    Constructed as an UH-1H-BF by Bell at Fort Worth, Texas, USA.

    October 1968

    Taken on Strength/Charge with the United States Army with s/n 67-17832.

    November 1968

    Transferred to 187th Assault Helicopter Company, Tay Ninh, Vietnam.

    13 July 1969

    During resupply mission the aircraft received five hits from small arms fire, causing minor damage.

    14 July 1969

    While on a smoke run the aircraft received five hits from small arms fire, causing minor damage.

    22 August 1969

    During cruise flight the aircraft received two hits from small arms fire, causing minor damage.

    16 October 1969

    While on a smoke ruin the aircraft received 12 hits from a 2.75 rocket. No injuries.

    October 1969

    Transferred to Army Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.
    For repair of combat damage.

    November 1969

    Transferred to 187th Assault Helicopter Company, Tay Ninh, Vietnam.

    12 November 1969

    While hovering over elephant grass the tail rotor struck an unseen stump causing a failure of the tail rotor. Hovering autorotation resulted in no further damage, all crew saved. Repaired in theater.

    1971

    Received extensive combat damage.

    Flew 73,617 hours with 187th AHC, with majority of missions being laying smoke screens.

    December 1971

    Transferred to ARADMAC, NAS Corpus Christi, TX.

    December 1972

    Transferred to 101st Airborne Division, Fort Campbell, KY.

    December 1975

    Transferred to Illinois Army National Guard.

    By November 1987

    Transferred to 44th Aviation Battalion (ARNG), Decatur, IL.

    By May 1997

    Transferred to E Company, 1st Battalion 106th Aviation Regiment (ARNG), Peoria, IL.

    By August 1998

    Assigned to Midway Airport, Chicago, IL for storage.

    September 1998

    To Prairie Aviation Museum, Bloomington Normal Airport, Bloomington, IL.
