    Taken on Strength/Charge with the United States Air Force with s/n 66-7647.

    Converted to an EF-4D.
    Modified as EF-4D Wild Weasel flak suppression aircraft. This is one of only two F-4Ds converted to EF-4D standard.

    To 6512th Test Squadron, Air Force Flight Test Center (AFFTC), Edwards AFB, CA.

    By 1980

    Redesignated as F-4D-30-MC.

    By 1980

    Transferred to 428th TFS, 474th TFW, Nellis AFB, NV.

    Transferred to 141st TFS, 108th TFG, New Jersey ANG, McGuire AFB, NJ.

    Transferred to 160th TFS, 187th TFG, Alabama ANG, Montgomery Regional Airport, AL.

    By 1985

    Transferred to 179th FIS, 148th FIG, Minnesota ANG, Duluth IAP, MN.
    Operated with markings: MINNESOTA, DULUTH
